OCF-3: Disability Certificate
What is the Disability Certificate (OCF-3) ?
The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation for your injuries. The OCF-3 form also known as the Disability Certificate is a form that needs to be completed in full and submitted to your auto insurance provider if you were involved in an accident on or after November 1, 1996. Claimants are required to fill out Parts 1 to 3 and then have their health practitioner fill out the rest of the form and return it directly to the insurance company. Only authorized health practitioners such as chiropractors, dentists, nurse practitioners, occupational therapists, optometrists, physicians, physiotherapists, psychologists and speech-language pathologists can fill out this form.

What information is required on the OCF-3?
Claimants are required to fill out Parts 1 to 3 which include the applicant’s personal information, insurance company information as well as a description of the accident – and then signed by the claimant. It’s important to note here that you do not sign anything that you are unsure of. For anything you do not understand are require clarification on – we recommend you contact a personal injury lawyer. The remainder of the form needs to be filled out by an authorized health practitioner and will ask them to outline any injury and sequelae information, disability tests and symptoms, if there are any previous examinations, investigations or consultations not previously reported by the health practitioner, if any further examinations are contemplated or required, prior and concurrent conditions, information on pre-existing disability benefits, pre-existing conditions or injuries, medications and other information related to the claimant’s health.
