OCF-2: Employer’s Confirmation Form


What is the Employer’s Confirmation Form (OCF-2) ?

The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation for your injuries. The OCF-2 form also known as the Employer’s Confirmation Form is a form that needs to be completed by you and your employer(s). The employer is responsible for submitting the form directly to your insurance provider. Lost wages can result in significant financial loss and reduced quality of life for individuals who have been injured in an accident. The OCF-2 form is required in order to obtain income replacement benefits to help reimburse insured victims for a specified percentage of lost wages.

What information is required on the OCF-2?

In addition to including your personal information – the form asks you to authorize your employer to disclose personal information to your insurance provider including copies of relevant documents directly relating to your application for income replacement benefits and any details regarding collateral sources of income or benefits. The insurance company will also ask for salary information, whether you are employed or self-employed, your income including any tips, commission or bonuses, your attendance history, whether you are entitled to receive income continuation benefits or supplementary medical rehabilitation or attendant care benefits, if you are a member of a union, whether you contribute to the Canada Pension Plan and other important employment details. It is important to note that if this information is not filled out correctly – it can result in drastically reduced or delayed compensation or benefits for your injuries.
