What is the Accident Benefits Application Package (OCF-1) ?
The Financial Services Regulatory Authority of Ontario (FSRA) has a series of Auto Insurance Claims Forms (OCF Forms) that need to be filled out if you are seeking compensation for your injuries. The OCF-1 form also known as the Accident Benefits Application Package is a form that needs to be completed in full and submitted to your auto insurance provider if you were injured in an automobile accident on or after November 1, 1996. Claims for certain accidents need to be made within 7 days. In addition to filling out this form, your insurer may also ask that you accompany this application with additional forms such as OCF-2, OCF-3, OCF-5 and/or OCF-23. However, you should not send in the OCF-5 form prior to speaking to a lawyer.
